practitioners area - how to refer

To refer a patient simply fill in the referral proforma from the link below. The referral proforma can either be printed out and faxed, or posted to us at our address. To comply with current data protection regulations we advise against e-mailing any patient information.

Once we have received the referral form we will contact the patient directly and treat as requested. On completion of the referral request your patient will be returned to you for continual care. It is our strict policy that on completion of the referred treatment, no referred patient will be seen again without an express request from their referring dentist.
